Best Facial Plastic Surgeon in Cleveland: What to Look For

If you’re researching facial plastic surgery in Cleveland, you’re probably discovering that you have a lot of options. Cleveland has excellent medical institutions and a strong community of surgeons, which is great for patients, but it can also make the decision feel overwhelming.

How do you know who’s the right fit for you? What do the credentials actually mean? What should you be looking for beyond a polished website and impressive before-and-after photos?

As a facial plastic surgeon who has spent my career focused exclusively on the face and neck, I believe the best thing I can do is help you understand what to look for, in any surgeon, including me. An informed patient makes better decisions, and better decisions lead to better outcomes. So here’s an honest guide to choosing the best facial plastic surgeon for your goals.

The First Question: Facial Plastic Surgeon or Plastic Surgeon?

This is where most patients start to get confused, and understandably so. The terms sound similar, but they represent two distinct training paths and two different areas of expertise.

A plastic surgeon certified by the American Board of Plastic Surgery (ABPS) has comprehensive training across the entire body, such as breast surgery, body contouring, hand surgery, burn reconstruction, and facial procedures. Their training is broad by design. Some plastic surgeons go on to specialize in the face (craniofacial fellowship), while others focus primarily on breast and body work.

A facial plastic surgeon certified by the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS) has followed a different path. These surgeons typically complete a residency in otolaryngology (head and neck surgery) which provides deep training in the anatomy and function of the face, nose, and neck. They then pursue additional fellowship training and board certification specifically in facial plastic surgery. Their entire career is focused on one area of the body: the face.

facial plastic surgeon vs plastic surgeon

Neither path is inherently better than the other, both produce highly qualified surgeons. But the distinction matters, because the face is unforgiving. It’s the MOST visible part of your body. There’s no clothing to cover the results. The margins between a beautiful outcome and a noticeable one are often measured in millimeters. For that reason, many patients specifically seek out a surgeon whose training, experience, and daily practice are concentrated entirely on the face.

I’ve written a more detailed guide on the certification landscape, including how to verify any surgeon’s credentials yourself, here: How to Choose a True Board-Certified Facial Plastic Surgeon.

What Does “Double Board-Certified” Actually Mean?

You’ll see this term on a lot of surgeon websites, including mine. Here’s what it means and why it matters.

A double board-certified facial plastic surgeon holds certification from two separate boards. In my case, I am certified by both the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS) and the American Board of Otolaryngology — Head and Neck Surgery (ABOto). This means I completed a full residency in head and neck surgery (5-6 years), then pursued additional fellowship training(1-2 years) and passed a rigorous examination focused exclusively on facial plastic and reconstructive surgery.

The ABFPRS certification, in particular, requires more than just passing an exam. Surgeons must submit operative reports documenting at least 100 facial plastic surgery cases performed as the primary surgeon within a two-year period, plus undergo peer review of their work. It’s a credential that demonstrates not just knowledge, but hands-on experience and ongoing commitment to the specialty.

When you see “double board-certified” on a surgeon’s website, ask: certified by which boards? The specific boards matter more than the number.

Questions Every Patient Should Ask

Board certification tells you that a surgeon has met a rigorous baseline standard. But it’s the beginning of your evaluation, not the end. Here are the questions I’d encourage you to ask any surgeon you’re considering (including me):

  • What percentage of your practice is facial surgery? A surgeon who performs face, breast, body, and hand surgery may be excellent at all of them, but a surgeon whose entire practice is faces is going to have a different depth of experience. Ask how many of the specific procedure you’re considering they perform each year.
  • Can I see before-and-after photos of patients similar to me? Not just the best results — ask to see a range. Look for patients with a similar age, skin type, and starting anatomy to yours. Consistency across multiple patients is more meaningful than one spectacular outcome.
  • What is your approach to natural results? Every surgeon defines “natural” differently. Ask them to describe their aesthetic philosophy. Do they talk about facial harmony and proportion? Do they listen to what you want, or do they push you toward a standard template? The answer tells you a lot about whether you’ll be happy with the outcome.
  • What should I realistically expect? A surgeon who is honest about what a procedure can and cannot achieve is a surgeon you can trust. Be cautious of anyone who promises dramatic transformations without discussing limitations, healing timelines, or the possibility that a result may differ from your mental picture.
  • What happens if I’m not happy with my results? This question tells you about a surgeon’s follow-up care and their willingness to take responsibility. Revision rates, how they handle complications, and how long they continue to see you after surgery are all important.
  • What is the full cost, and what does it include? A trustworthy practice gives you transparent pricing that covers the surgeon’s fee, anesthesia, facility costs, post-operative care, and follow-up visits. Be wary of quotes that seem unusually low or that don’t account for the full scope of care.

How to Evaluate Before-and-After Photos

Before-and-after galleries are one of the most important tools in your research. But not all photo galleries are created equal, and knowing how to read them will help you evaluate any surgeon more accurately.

Look for consistency, not just highlights. Any surgeon can curate their five best outcomes. A gallery that shows a range of patients with a range of starting points, and consistently achieves natural, balanced results across all of them, tells you more about a surgeon’s skill than a handful of dramatic transformations.

Check the photo quality and angles. Consistent, standardized lighting and angles (front, three-quarter, and profile views) allow you to make a fair assessment. Be cautious of photos taken at different angles or with dramatically different lighting between the before and after — that can make results look more impressive than they are.

Pay attention to the “after” results over time. Early post-operative photos can look different from fully healed results. For rhinoplasty, final results take up to a year. For facelifts, results settle over several months. A surgeon who shows longer-term follow-up photos is showing you honest, stable outcomes.

Look for results that match your goals. Do their results look natural? Do the patients still look like themselves? If the gallery is full of results that look overdone, tight, or generic, regardless of how technically proficient they may be, that’s a signal that the surgeon’s aesthetic preferences may not align with yours.

Red Flags to Watch For

While most surgeons in the Cleveland area are qualified and ethical, there are some warning signs that should give any patient pause:

  • Aggressive sales pressure. A consultation should feel educational, not transactional. If you feel rushed toward a decision, pushed toward more procedures than you came in for, or pressured to commit with a limited-time offer, that’s a red flag.
  • Guaranteed results. No ethical surgeon guarantees a specific outcome. Surgery involves your unique anatomy, your healing biology, and variables that no one can fully control. A surgeon who guarantees results is overpromising, and overpromising erodes trust.
  • Lack of transparency about credentials. If a surgeon is vague about their board certification, training, or experience when asked directly, that’s worth noting. Qualified surgeons are happy to explain their background in detail.
  • No mention of risks or limitations. Every procedure has risks, limitations, and trade-offs. A surgeon who only talks about the upside is leaving out half the picture. You deserve the full truth.
  • Unusually low pricing. Facial surgery is a significant investment, and the cost reflects the surgeon’s experience, the quality of the facility, the anesthesia team, and the level of post-operative care. A quote that seems too good to be true often means corners are being cut somewhere.

Why the Consultation Matters More Than Anything Else

You can research credentials and study galleries online, but the single best way to choose a facial plastic surgeon is to meet them in person. A consultation is your opportunity to evaluate the entire experience, not just the surgeon’s qualifications, but their communication style, their honesty, and whether you feel heard and respected.

In my practice, a consultation is a conversation. I want to understand what brought you in, what you’re hoping to achieve, and what’s important to you. I examine your facial anatomy, explain what I see, and discuss what’s realistic. If a procedure isn’t the right fit for your goals, I’ll tell you. If there’s a better option, I’ll recommend it. The goal is clarity — for both of us.

I always tell patients: you should leave a consultation feeling more informed and more confident in your decision, regardless of whether you choose to proceed. If you feel confused, pressured, or dismissed, that surgeon isn’t the right fit for you.

About Dr. Diana Ponsky

I wrote this guide to help you make the best decision for yourself, regardless of which surgeon you ultimately choose. That said, I’d like to share what I bring to this work and why I believe my practice is a good fit for patients seeking natural, personalized facial rejuvenation in Cleveland.

My path to facial plastic surgery started early. I came to the United States as a child with my family, refugees from Vietnam who spent a year in an Indonesian refugee camp before arriving in America. That experience shaped everything that followed, including a deep work ethic, an appreciation for opportunity, and a drive to build something meaningful. I earned a full scholarship to New York University, then attended Georgetown University School of Medicine, where I completed my residency in otolaryngology (head and neck surgery) and was honored with the Roy Sessions Exemplary Chief Resident Award in 2005.

After serving at the Veterans Administration Medical Center in Washington, D.C., I moved to Cleveland to pursue fellowship training in facial aesthetic and reconstructive surgery under Dr. Bahman Guyuron at Case Western Reserve University, one of the most respected rhinoplasty surgeons in the world. That fellowship is where I refined my approach to rhinoplasty, facelift, and the full spectrum of facial rejuvenation.

Following my fellowship, I was invited to join the departments of Plastic Surgery and Otolaryngology — Head and Neck Surgery at University Hospitals — Cleveland Medical Center, where I served as Director of the Division of Facial Plastic and Reconstructive Surgery and as an Assistant Professor at Case Western Reserve University. In 2015, I opened Ponsky Facial Plastic Surgery in Beachwood to create the kind of practice I believe patients deserve: boutique, personal, and focused entirely on the face.

I am double board-certified by the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS) and the American Board of Otolaryngology — Head and Neck Surgery (ABOHNS), and I am a Fellow of the American College of Surgeons (FACS). I have performed more than 1,600 rhinoplasties and have over 18 years of experience dedicated exclusively to facial plastic surgery. My entire practice, every surgery, every consultation, every treatment, is focused on the face and neck.

My work has been recognized by my peers and my community in ways I’m deeply grateful for:

Beyond my practice, I’m passionate about teaching and giving back. I continue to mentor residents through a formal rotation at my practice, speak at national meetings including the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS), and have published peer-reviewed research in major journals on rhinoplasty and facial aesthetics. I’ve been featured in national publications including Redbook and Allure, and on Cleveland’s WKYC and Fox8 Cleveland.

I also believe deeply in using my skills to help others. I provide pro bono facial plastic surgery to domestic violence survivors and women living in poverty through the FACE to FACE program, and I’ve traveled internationally with Operation Smile to perform cleft surgeries in underserved communities. Closer to home, I volunteer with MedWish and Medworks in Cleveland and regularly mentor young women interested in STEM and medicine.

My team includes Olivia Marino, NP, our aesthetic nurse practitioner, and Mary Bastulli, RN — both of whom share my commitment to patient care and natural results. We serve patients from Beachwood, Shaker Heights, Pepper Pike, Mayfield Heights, Solon, Lyndhurst, Lakewood, and throughout Northeast Ohio. For surgical procedures, we’re honored to care for patients who travel from across Ohio and neighboring states.
